 Job Summary: We are seeking a friendly and compassionate Psychiatric Nurse Practitioner to join our growing team. In this role, you will provide comprehensive care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. This is an exciting opportunity to make a meaningful impact while working in a supportive and collaborative clinical environment focused on improving respiratory outcomes and enhancing patients’ quality of life. 

Responsibilities: 

  • Recommend psychiatric medication and treatment recommendations.  
  • Integrate pharmacological and behavioral interventions, emphasizing non-pharmacological treatments whenever possible. 
  • Develop and implement individualized treatment plans  
  • Collaborate with interdisciplinary teams  
  • Educate patients, families, and facility staff on cardiovascular conditions, cardiac treatment plans, disease prevention, medication management, and continuity of care. 
  • Monitor patient progress and adjust care plans as needed  
  • Maintain accurate and timely EMR documentation  
  • Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events 

 Requirements 

  • Master of Science in Nursing  (MSN) 
  • PMHNP  
  • Optional Doctorate in Psychiatry  
  • Active Psychiatric Nurse Practitioner License  
  • ANNC Board Certification  
  • Excellent communication and documentation abilities 
  • Strong patient assessment skills 
  • Strong communication, organizational, and teamwork skills  
  • Ability to work independently in the clinical environment 
  • DEA
